Responsibilities
You are responsible for providing complete and accurate
information about current and past illnesses. You are responsible
for reporting perceived risks in your care and unexpected
changes in your condition. You should ask questions if you
do not understand information or instructions. You should
tell your physician or nurse if you cannot perform any of
the tasks asked of you.
You are responsible to follow the rules and regulations
of the hospital and for respecting the rights and needs of
other
patients, visitors, and staff. Among these is compliance
with the requirement that the hospital remain a smoke-free
environment.
You are responsible for providing insurance information
to the hospital and for working with the hospital to
arrange payment.
It is your responsibility to provide the hospital with a list of all medicines that you are currently taking. This list needs to include all prescription medicines, over the counter medicine, and any vitamins or herbal remedies you are taking. This can be a written list or your bottles of medicine.
You are responsible for your own lifestyle and how it
impacts your health.
