• Evaluated on a case by case basis
• Short term prognosis due to any condition
• Not yet ready for Hospice services

Services Provided:

• Palliative Care Nurse will assist with symptom management
• Music Therapy offered to patient and family
• Volunteer services available for respite and support
• Chaplain services by hospitce trained personnel
• Bereavement services available to the family

Inpatient Palliative Care

• Provided at no additional charge to the patient
• Provides additional supportive services while in hospital
• Provides patient/family with a contact person should they become hospice appropriate

The Referral Process

• MD order obtained for hospitce/palliative care consult
• Liaison nurse reviews the medical record and meets with the patient, family and case manager
• If appropriate, palliative care initiated
• If hospice appropriate, refferal made
